Enrollment Documentation Requirements
Preschool Requirements if child is screened and qualifies:
Certified Birth Certificate is due within 30 days of enrollment
Up-to-date Kentucky Immunization Certificate is due within 2 weeks of enrollment
Current Physical Examination (dated within 6 months prior) is due within 2 weeks of enrollment
Vision Examination by optometrist or ophthalmologist due by January 1st of current school year
Proof of Residence in the name of parent/guardian – 2 forms of proof are due before starting enrollment
Kindergarten Requirements:
Must be 5 years old on or before August 1st of the given school year
Certified Birth Certificate is due within 30 days of enrollment
Up-to-date Kentucky Immunization Certificate is due within 2 weeks of enrollment
Current Physical Examination (dated within 1 year prior) is due within 2 weeks of enrollment
Vision Examination by optometrist or ophthalmologist due by January 1st of current school year the first time a 3, 4, 5, or 6 year old is enrolled
Dental Screening or Examination by a dentist, dental hygienist, physician, registered nurse, nurse practitioner, or physician assistant due by January 1st of current school year the first year a 5 or 6 year old is enrolled
Proof of Residence in the name of parent/guardian – 2 forms of proof are due before starting enrollment
First Time Enrollees in KY Schools Requirements:
Certified Birth Certificate is due within 30 days of enrollment
Up-to-date Kentucky Immunization Certificate is due within 2 weeks of enrollment
Current Physical Examination dated within 1 year prior is due within 2 weeks of enrollment*
Vision & Dental Examination if age appropriate
Proof of Residence in the name of parent/guardian – 2 forms of proof are due before starting enrollment
Students Entering 6th Grade Requirements:
Second Physical Examination dated within 1 year prior to entering 6th grade
Up-to-date Kentucky Immunization Certificate including first dose of Meningococcal vaccine prior to entering 6th grade
*Physical exams not on a KY form must include the following:
Medical History
As assessment of growth, development and general appearance
A physical assessment including hearing and vision screening
Recommendations to the school regarding health problems that may require special attention in the classroom or physical education activities
